I hope what I am offering answers things.
The development of ones craft over time, "the work of ones hands" (from music to plumbing) is the internal validation. The external validation feeds very strongly into the act of giving. Their is nothing makes a person happier than to give, commensurate with the degree of intimacy one can understand (the content of a song).
Once one sheds, through the development of ones craft (to a degree, we are not islands, we need people) the doubts as to weather one is; relevant, useful, functioning, employable, and all the millions of things like that - then you can choose to "give" you are no so caught up with the worries that it takes to get to that stage. And to be able to "choose to give" makes the giving even better.
Being an artist is being true to / connecting with yourself in your performance, how it suits you at the time, but entertaining is paying attention to the people in the room; giving to, and caring about them.
This book is for the frontman, bandleader, or soloist. Someone who takes responsibility for the flow, pace, structure, material selection, engagement, and many dynamics relating to audience relationship. It addresses the need to be concerned with the way something is delivered as much as what and where it is being delivered.
